Critical Friends Network (CFN) Application to become a Critical Friend
Involving our employees, patients and members of the public is vital when we are looking to develop and improve the care and services we offer.
This year we are keen to re-establish a Critical Friends Network (CFN) to hear the patient and public voice and to build this into everything we do.
Members of the CFN will have the opportunity to contribute towards service developments and improvements either by attending focus groups and meetings or by sending feedback virtually via email, survey or telephone. CFN members should have some experience of using Yorkshire Ambulance Service NHS Trust (or any of our services, for example NHS 111 or our Patient Transport Service) within the last three years and should have a keen interest in helping to develop our services and how we deliver these in the future.
If you would like to be involved please complete the application below.
By completing this online form you are consenting to become part of YAS’ Critical Friends Network (CFN) and will be invited to participate in projects or work streams that are underway in relation to the areas of interest outlined above (This might be involvement in a similar role in another NHS Trust, or you are paid to provide services to YAS.)
You also give consent by signing this form to Yorkshire Ambulance Service NHS Trust holding your contact details and sharing these where appropriate with relevant agents such as the YAS Expert Patient who helps coordinate many events. Your information will never be shared inappropriately. We comply with the Data Protection Act 1998 and take all reasonable measures to keep your personal data safe at all times.
